We appreciate your patience and support as we have worked through the closing of our hospital. We know many of you have reached out several times and are waiting for responses.
Due to the extreme volume of records requests, please allow up to 7 to 10 business days for us to get back to you. To help us help you faster, we also ask that you refrain from leaving multiple voicemails or emails for the same request.
We sincerely apologize for this additional wait time and promise that we are working hard to respond as quickly as possible.
In the meantime, we ask that you please do not call or email Crestwood Animal Hospital with records requests, as they do not have access to our client records and will not be able to assist you.

To our Wellness Plan clients, please check your email for a communication that went out on 4/27/22 with more information. If you have not already reached out to us about your options, you will be receiving a letter in the mail with that information. If you would like to expedite the process, please email us or call the number above to let us know how you would like to proceed.
We know that having your pet’s veterinarian close unexpectedly creates a challenge in continuing their care with someone you trust. We are working within our network of sister veterinary clinics in the Greater Louisville area to secure a welcoming experience for you and your pet(s) to help further ease this transition. Please be on the lookout for an email for more specifics on these clinics and the welcome offers they will be extending to you.
